Self Referral

If you have a query regarding sickness absence, planning a return to work, or guidance regarding requirement for adjustments in the workplace, it is advisable to discuss these issues with your manager in the first instance and to request a referral to occupational health to encourage collaborative working.

 

There are two methods of referring to the occupational health service:

 

Through your line manager

 

You can discuss with your line manager and ask them to make a referral to the occupational health service for you. Your manager should give you a copy of the completed referral form.

 

Self Referral

 

You can contact the occupational health service yourself via the self referral form. Self-referral is confidential; therefore your line manager would not be contacted without your knowledge and consent. You are however encouraged to report any work-related health problems to your line manager, so they can support you appropriately.
