Now that you have been made a conditional offer of employment with NHS Grampian we wish to know about any disabilities or health conditions that may affect you in your new post so that, where appropriate, we can advise your manager of any support needs you may have and how these can be provided for. We also need to ask about conditions which could pose risk to patients so that we can assess these and consider how any risk can be avoided, whilst assisting you to successfully carry out your role with NHS Grampian. This can be achieved with most health problems and disabilities, even if substantial.
As a prospective employee of NHS Grampian you have a duty to provide relevant information to GO Health Services. Failure to disclose information about a physical or mental health problem that could affect patient safety would be a breach of this duty and could result in disciplinary action.
The information provided will be retained by GO Health Services and will be held on a secure database and will not be used for any other purpose without your consent. Please note however some information may be shared for statistical purposes only which will not identify individuals.
Your manager will only be informed of the effects of a health problem or disability, if relevant to your needs or patient safety, and of recommendations on support or adjustments that could be of assistance to you.

ONLINE PRE-PLACEMENT HEALTH QUESTIONNAIRE
Complete the online Pre-Placement Health Questionnaire as soon as possible (make sure to select ‘NHS Grampian’ as the organisation you will be working for).
All staff who have been conditionally offered a clinical role are required to provide a completed Immunisation Record which must be stamped and signed by your GP/Practice Nurse OR a printed copy of your GP immunisation record which clearly states your name and date of birth. We also accept stamped and signed records from a previous OHS provider.
*** In order to know whether an appointment is required if you could please scan and email (or post) this completed immunisation record to gram.ohs@nhs.scot a.s.a.p. When submitting vaccination evidence please ensure that this is provided in English and that you use the immunisation record form provided. ***
