GO Health Services are contracted to support the Occupational Health of staff working in Dental Practices in Grampian who offer NHS work. Please choose from the options below depending on what type of Practice you work for:
- NHS GRAMPIAN SALARIED PRACTICES should use the regular paperwork available from our intranet pages (available only for NHS Grampian Managers and Staff).
- NON NHS GRAMPIAN SALARIED PRACTICES should use the paperwork provided below:
Practice Managers and Dental Staff can now access the necessary paperwork for EPP Clearance and for Immunisation Clearance/TB Screening. It is important that all the paperwork is completed. Please read the Explanatory Notes and Information Sheet below and then complete the Questionnaire to initiate the screening process.
